How consent is collected

When a patient books or checks in for an appointment, a trained staff member asks whether the patient would like to receive text-message reminders for upcoming appointments. The staff member reads the script below word-for-word. The patient's verbal response ("yes" to enroll, or anything else to decline) is logged in the practice's scheduling system along with the date, time, and the staff member's name.

No patient is enrolled without an explicit verbal "yes" captured on the spot. If a patient asks questions or declines, the staff member answers the questions and does not enroll the number.

Verbal consent script (read to patient)

Staff reads verbatim
"As part of our appointment management, we'd like to send you text-message reminders for your upcoming appointments with your eye care provider. You'd receive up to 4 messages per month. Message and data rates may apply. You can reply HELP any time for assistance, or reply STOP to unsubscribe at any time. Shall I enroll your mobile number in these reminders? Please say 'yes' to confirm — or let me know if you have any questions."

Consent is only recorded on an explicit "yes." Silence, non-answers, or "maybe" responses are treated as a decline, and the number is not enrolled. Staff logs the consent decision (yes or no), the date/time, and the staff member's name in the practice's scheduling system.
